LIFE CERTIFICATE FOR PENSIONERS
Service of Life certificate for pensioners is provided at Consular Section, Consulate General of India, Houston. It is also provided through VFS Global (https://services.vfsglobal.com/usa/en/ind/miscellaneous-services)
How to apply by postal mail at Consular Section, Consulate General of India, Houston:-

Download the Miscellaneous Application form, fill it up carefully and affix his/her photograph on the same. ONLY ONE COPY OF LIFE CERTIFICATE WILL BE ISSUED. |

Add a recent (no later than 15 days) certificate from a registered medical practitioner, certifying that applicant (with name) has been examined (with date) by the medical practitioner and is alive on the day. |

Applicants for pension purpose may submit notarized copy of the pension book (having name of the applicant)/bank documents of pension, only in case applicants want PPO number to be written in the Life Certificate. |

Original and one photocopy of the Life Certificate (dully filled in and signed by applicant), which has been provided by the Pension Authorities in India. In the absence of this document, Embassy will issue Life Certificate as per its own format. |

Notarized copy of applicant’s current valid passport and its photocopy. Those having Indian passport may provide notarized copies of the first and last pages of the passport. Original and photocopy of the documents showing valid US visa status. |

Original and photocopy of any of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card or Mortgage Deed. |

Please send in the application enclosing a trackable pre-paid self-addressed return mailing envelope to 4300 Scotland Street, Houston, Texas-77007. |

Fee: (a) Life Certificate for pension for applicants with Indian and non-Indian passports. A fee of US$2.00 (Two Only) towards Indian Community Welfare Fund (ICWF) per application. The fees may be paid as money order/cashiers’ cheque, drawn separately towards ICWF, in favor of Consulate General of India, Houston. |

Processing Time: Ten working days from the date of receipt of mail at the Consulate, subject to submission of all requisite documents. |
Note:- If applicant is coming in-person, please come between 10:00 AM to 12:30 PM to the Consular Section, 4300 Scotland Street, Houston, Texas-77007. Medical certificate and notarized documents are NOT required for applicants coming in-person to the Consulate. Please bring original passport and proof of residence along with Misc. Application Form. The application would be processed on the same day.
